Service Description

Thank you for referring your patient. We are a psychiatric medication management office. We specialize in depression, anxiety, ADHD/ADD, PTSD, OCD, bipolar without psychosis, borderline personality disorder, stress, obesity, and insomnia. We DONOT accept schizophrenia, dementia, psychotic bipolar, Alzheimer's, and or patients not invested in improving their overall health. To make a referral please fax clients most recent note, medication list, and chief complaint to 336-234-1002. We will review and call the patient to schedule if we are able to accommodate them. We NO longer accept patients under the age of 14. Please know if you are referring the patient to this practice that doesn't automatically give our providers the legal means to send a copy of medical records. Please send a signed release form with your recent note if you are wishing to obtain it.
