Service Description

If follow up from initial exam is needed the cost is $35 plus $30 travel fee. For home visits there will be a travel fee of $30 plus the exam fee $110. Travel radius must be within 25miles from provider location.

Cancellation Policy

We require 48 hours in advance to change or cancel appointments. A fee of $90 will be charged for same day cancellations. Insurance does not cover this fee.
